Renting part of house

I have a 2 bed semi-detached house in Guildford (1 1/2 mile from City Centre in a residential area with no LA housing) which I don't live in, but my son still does.

I rent a double bedroom + ensuite with shared use of living/dining/kitchen/garden and use of a parking space.

My current lodger is vacating this weekend and I wanted to just sense check a few things

Does anyone else rent out a property with this scenario? (and similar City demographic!)

If yes

What rent do you charge?
What type of agreement do you use?
What do you include/exclude - bills wise?

I included everything except phone as I thought this was easier but according to others who I have spoken to I think I've probably been too reasonable.
