Doctor in the house PLEASE!! Re high dose methotrexate ALL
Discussion
I was suggested to come here via the general gassing station and I appologise to the moderaters if they move it! Here it is ...................
Hi everyone sorry for being a pain. I would just like to know if anyone knows what the side effects of 120mg of Methotrexate within 24 hours will do to a 10 year old child. His Neutraphil count is 1.30 will this make him netrapenic with in a few days? And what are the other side effects. I have been given a regime of what I need to give him in the next 24 hours and then folinic acid after 48 hours after the 1st dose. Is there anything I should look out for? He has had VRE and have been told he is on isolation for the rest of the treatment (2.5 years) I Know the complications with this! Whoops sorry he has testicular relapse of Acute lymphoblastic luekemia cancer. He has had the intesive treatment which they could not finish due to the hospital resistant bug VRE and recently had 3 weeks of everyday radiotherapy.
Doctors are not that receptive to questions!!!!!
So I would be greatful if any of you know. We would so love to go out over the bank holiday but don't want to if he becomes netrapenic and will pick up a bug in 2 seconds flat! We know if he gets a bug it will spure off the VRE and could kill him so we have to be careful. Are we house bound again!!???
Thank you everyone XXXXXXXXXXXXXXXXXXXXXxxx

Doctors really bl**dy should be approachable enough to cover that, you shouldn't have to ask us motley crew for answers to such crucial questions.
I'd suggest ringing his paediatric oncology ward and a) asking the nurses or b) getting the nurses to get hold of a doctor for you.
If in doubt, play it safe.
Best of luck!

Frankly with a neutrophil count of under 2, I wouldn't take him anywhere he could get a bug anyway.
I'm no doctor and I was never given methotrexate myself as a patient, but I imagine it will knock his neutrophils somehow. I've just had a read of the BNF and it mentions that neutropenia is a less common side effect, but still possible.
I wouldn't risk it. A day out isn't worth him catching an infection that he isn't very well equipped to fight.
Surely you can ring the unit that are looking after your little 'un? Getting unqualified advice with such a serious thing is madness. I know my Doc's were extremely receptive to questions and someone was available 24hrs a day. You might not like their answers, but I'd be very surprised if they tried to put you off gleaning any information, that would be extremely unprofessional.
I imagine the Doc's will default to NO in this case though. I would if I was them. I know how dodgy it was when I was neutropenic and how easy it was to pick up a minor bug. Having seen 4 people I knew die on the ward thanks to infections picked up whilst neutropenic, I'd say don't do it.
