Movie took place in a small town, 2 brothers V a low level local gang of small time drug pushers.
Sane brother takes it upon himself to execute everyone, I think. Possibly for the bullying of his not to bright brother.

It was a semi-dark sort of movie.
IIRC the protagonist get the thugs high, imagining things. May have worn a gasmask, making them think they imagined his passing, or some similar cobblers...
