Replacing cup+cone BB

My BB has started making a nasty grinding/rumbling noise.
It's a Raleigh bike from 2003 with a cup+cone BB. I could replace the bearings but I wonder if the shells are worn now.
How easy is it to replace with a cartridge BB? I have access to all the tools, but no idea if they just screw in to the same threads or what.
I also read up that Raleigh do funny thread sizes but not sure if this is only for much older bikes.

I don't know what bikes you're riding Neil but mine don't have hubs in the bottom bracket.
It should be a straight swap. Shimano still make their UN range of square taper bb's a UN54 will give you good service. Pop into your lbs with the bike - or the removed bb - to make sure you get the right size.

tsocks, I completely misread the OP 
As Mk1fan says, simply replace with one the models from the UN50 range (shops may still have UN51's UN52's etc in stock).
Usually about £14 quid, and easy to fit.
Well bought a new cartridge BB.. one of the ball races had bent the cage and the balls were just floating loose in the BB.
Getting the fixed cup out defeated me! Whoever designed it so that you had about 3mm to grip with a ridiculously big spanner needs shooting. Can't get any leverage on it because it keeps slipping off..
Any tips? Sheldon Brown recomends a) using a vice or b) making up a tool with a bolt and a nut that bolts through the centre and is used to clamp the cup, anyone tried this?
Otherwise it's back to the LBS, but they're booked up for most of the month
