I recently heard of someone who destroyed his radiator by dropping the rad cap into the fan whilst the engine was ticking over. Ouch!
On my cars I always fit a piece of 'toilet-type' chain to the cap with a pop rivet, then fix the other end of the chain to the web on the body just in front of the wiper motor with a 2BA nut and bolt.
