Part number EAC 1658 PA?

Good afternoon, i'm trying to locate another of these as i've just purchased 2 from the states and i need three for a triple plenum set up. Does anyone know which model they are off and secondly does anyone have one spare to sell? By the way i tried to attach a photo but can't seem to do it, Cheers Jonathan
