I had a friend years ago who had a pretty impressive miniature railway which worked - the tracks must have been milimetres appart.

Can anyone explain guage sizez? Im curious how small the smallest mini steam locomotive is.

Steamy website says... said:

This tiny engine actually runs! The device works much the same way as a normal steam engine. It has a tiny reservoir of water, and a boiler with a heater in it. When the tiny heater is turned on by applying an electrical signal, the water boils creating steam. The pressure generated by the steam forces the piston outward. When the electrical signal is removed, the steam condenses back to water, and the pressure is reduced, and the piston retracts back into the cylinder. By turning the voltage on and off, the piston repeatedly moves in and out of the cylinder.

I believe that I have seen an "N" Gauge live steam railway locomotive before. I think it was single cylinder with a flash steam boiler, so not really a scale model.

I guess that even the 5" and 7 1/4" engines are'nt that close in terms of scale, but the N gauge one did work.

I don't know if it's on the internet. I saw it in a magazine probably a decade before the internet existed.

Live steam in OO scale is in current production (Hornby), so one-offs in N sound quite possible.
