NHS pension 95 section

My wife has requested the 95 part of here pension , application was checked by her managers & ok,d but we have now received this letter .
1 . We believe the first one is that the person ( witness ) who filled the form in put her down as job fining 25/02/25 , and then job will start 25/02/25 , I believe
there has to be a break between those dates , day , week , we’re waiting to find out what that break is .
2. It then says your witness dates don’t match , but we can only find one date put in by the witness & that looks like his d.o.b , I’m assuming as the date he witnessed was the 25/02/25 that’s the date he should of put down ?
3. Confirm if you would like to take the additional lump sum , we put no for this on the application form ( sec 10.2 ) , but then we filled in the adjacent section b which should of been only if you’d ticked yes , the max tax free amount or
We tried to ring the pension dept , day after day & no one will answer the phone , does our reasoning sound right ? . Thankyou

It’s all very secretive the NHS pension you don’t really know what your getting at the end and there is a lot of people who don’t know the 95 scheme needs to be taken at 55 or 60 otherwise your losing out.

Up to 209,553 NHS pension scheme members could be missing out on their pension payments due to a lack of awareness around the recent pension scheme changes, a freedom of information request from Quilter has revealed.
From 1 April 2022, the old NHS pension scheme (final salary 1995/2008) was closed, and all active members were moved to the new the 2015 scheme.
Whilst members were unable to accumulate benefits in the old scheme after this date, a final salary link was established to ensure that any future increases in pensionable pay would still enhance the benefits from the old scheme.

Thankyou for links , yesterday hospital said wife had to have 2 weeks off after talking to pension dept , today pension dept rang her to check she was ok with what they’d put in letter ( fine ) , pension women said why have you put 2 weeks on form , you only need to have 24 hrs off ( confirmed by link above ) , so who did they speak to , or did they speak to anyone .
Hopefully sorted now .
