3.0tdi starts then dies

2008 audi a4 cab 3.0tdi died whilst driving and now won't run it starts but immediately dies again RAC man gave codes P1580 throttle control valve
P1440 egr valve open circuit and p0642 sensor reference voltage A circuit low.

Can anybody point to a fix from this....the egr code maybe from me disconnecting/reconnecting it during a coolant change at the weekend car drove fine after coolant change so hoping breakdown is a coincidence.
