Strange noise...

When I'm in the outer front ring there is a grinding type noise on the downward stroke of the left (nearside) pedal, not just under heavy pressure, anything more than just spinning the pedals with the wheel off the ground causes it.
I tweaked the outer limit screw out more last night, might try it the other way tonight, but I'm not expecting it to improve it due to what causes it.
Any suggestions?
No noise under at all when on the inner 2 rings, also makes no difference with what gear I'm in on the rear cassette.
So in my head it can't be any bearings as that would happen regardless of being in outer ring or not.

Weird that it's just on one pedal stroke!
Could it be a BB issue? Do the cranks move from side to side when you grab them and try to move them? That movement could cause the chain to rub on the mech cage or even on the rings themselves.
Only other pedal stroke related noises I've had are clicking - needed new bearings in pedal or squeaking where a dab of grease on the spds helped.
