Why do tyres suddenly bulge?

This morning I thought I had developed a particularly sensitive arse and could feel every bit of grit on the road. At lunch time I looked down to see a great big bulge in one section of the tyre, hence the wheel not running flat.

So why/how does this happen? The inner tube has no repairs in it, but from the outside it looks like it has twisted and turned a corner with the tyre trying to follow. So new tube and or tyre?

New tyre, the casing has started to break down.

Keep going and there'll be a bang to go with your bulge.
