My OH is looking for a new job after being in the same job for 20+ years. As such her CV is very short and I wonder if this puts off prospective employers.

She also would like to go into a different role, something that she did do in her old role but not to any great extent. Should we rewrite her CV to highlight this aspect of her role?

No reason not to show her progression within the old job or highlight key projects. Eg.

Company A, 1993 - Present

2011 - Present Department Head
2010 - 2011 Transformation Project Lead
2006 - 2010 Deputy Head
2001 - 2006 Associate

And no, a short CV won't put people off, but it will raise the question as to whether or not she is really ready to change, learn a new company, culture and way of doing things after that length of time. She will need to answer that in the interviews.

However if she has literally been doing the same job for that amount of time then people probably would question why she hasn't progressed in her career at all.

If she has been effective in her roles and can demonstrate impact along with a healthy diet of continuous learning, then a CV to capture that is not a problem. Deciding what she wants to do next and identifying whether she has the necessary competences is also very doable. If she has them, then apply away. If not, work out the delta and then decide if that can be filled someway. If not, change tac and head towards a more appropriate role. Good luck to her!
