Normally 'Joe' would be the tuner who over a period of time has experimented with said engine and found that a cam with a certain lift, overlap, duration etc etc works best for a certain vehicle or task.
Very rarely would he be a cam manufacturer. He just specifies the cam parameters and gets the cam manufacturer to grind it for him. The well known 885 cam would be an example. I don't know who the 'Joe' is in this case but the cam is made/ground by Kent Cams but you can't buy it from them as it is a custom grind.

For LS engines you can get a cam specified, you give them intake tract length, exhaust header length and diameter, gearing, weight, desired power band, even how you want the idle to sound, you then get a build sheet to give to the cam manufacturer.

Yes, ideally you want it to be the last thing you do, as you spec the cam (or have some one spec it for you) once you know where you are with all the bits around it, other things too, like valve sizes, intake and exhaust port sizes and volume...Here is my latest spec advise after i filled in a form, and he then specs a cam, costs $25.00..
ETA, but if your build is a known quantity, then they know what works already, but mine is a very unique engine, hence the bespoke cam.
