I've had an E220d with the standard controller since Sept 17.
The top picture is the optional 'Command' controller. IIRC this controls the optional wider screen that also has a better Nav system. The standard Nav system is garmin based and the graphics aren't quite as good. It's been fine, though. There were other 'toys' with the optional Command system but it was a £1600 option.
Personally, I think that the raised part above the whizz-wheel controller might get in the way. But then it might actually be a controller in itself too, I'm not sure.
The whizz-wheel controller has been really intuitive to use whilst driving - you just twist, up, down, left, right and push to get what you want. Easy.

The top controller is a touchpad and allows you to "write" on it. It's not particularly useful IMO, and it does get it the way of the wheel sometimes.
There are a few iterations of infotainment on the W213 range. COMAND Online is the highest spec and includes a 12.3" screen, the touchpad, Apple CarPlay/Android Auto and fancy online features like automatic map updates, petrol prices, parking space availability and some other stuff. The online features are all subscription based after 3 years, as are map updates and live traffic. An additional 3 years of map updates, live traffic and online services is about £450!
2016 - 2017 MY cars got the Audio 20/Garmin SD card based nav system with an 8" inch screen. It's rubbish - it's terribly slow and the routing is bizarre at times.
2018 onward cars get the 12.3" screen as standard with Audio20 and a much better SD card based nav system.
If you're looking at a 2016 or 2017 car, you really want one with COMAND Online. 2018 onwards it doesn't matter so much, unless you really want Apple CarPlay/Android Auto.

I have a 2018 E220d with a 12.3" screen, SD nav with routing, fuel prices, weather, traffic etc.
I don't have Apple car play - to enable it costs a billion pounds.
I've also got just the wheel controller, which I actually prefer to the one with the extra controller. It's nicer to use and looks better.
