Windows 11 lost password

I'm attending to a Lenovo Thinkpad E530 for a friend.
Basically, it got in a mess, and it didn't have any security installed. Actually, I'm sure it did have Windows defender or whatever it's called now; but when it was presented for a fix, it didn't have.
So - said I wanted to put the Windows Security on before I handed it back. But... it wasn't there. Not at all. The only way to recover it was to do a Windows Reset. which I did opting to keep the data but download the files. All went well.
Computer owner comes to pick up her computer, and we go through the process of adding a Windows password.
I take her through, and she repeats several times in front of me, shutting down and starting up and putting in her password at the opening screen.
She goes home happy. She starts her computer up at home and works. Until the next morning. The new password doesn't work, I know what it is so I check with her. Still can't get in, so the computer comes back to me. I've tried with chntpw via a Ubuntu startup stick, but couldn't get it to work. I'm now resetting Windows again.
What causes W11 to lose user passwords, and how do I prevent it happening again?
