The gear knob on my td5 came off the other day. There was a soft'ish plastic sleeve about 20mm long left on the threaded part of the gear stick, I thought I may be able to squeeze this back inside the gear knob and simply screw it back on, however this is not possible. It is a very tight fit and the sleeve would not fit back inside, it was so tight that it seemed surprising that it would have been possible to have the sleeve inside and screw it on.

I am guessing that if I buy a new knob (it is an XS so ideally will source a leather one) it will be in one piece and simply screw onto the gear stick thread?

Anybody else had this problem and/or got any experience of this?

Can you remove the sleeve and fit it into the knob? When you've done that put some liquid soap on the gear lever shaft and slide the knob on. The soap will act as a lubricant but will dry out and leave the knob stuck onto the shaft.
