I think it's simply a case of going into your profile and browsing the replacement photo. I seem to recall having problems with this on my last change, in that it didn't appear to have made any difference (despite several attempts), but when I came back some time later the change had actually been made.

Do a "ctrl + F5" refresh. your browser is getting the cached picture.
