Clutch Judder / Slip

I'm looking for advice on what I think is clutch-judder.
It's on a Nissan Primastar van (same as Vivaro / Renault Trafic) and since new, I've clocked up 13k miles. Living and working in London means I'm in quite a few slow traffic scenarios; lately, I've noticed that there is a judder / slipping feel to when I'm pulling off in first-gear; purging along with the masses is getting me worrried that my driving skills have deteriorated, or is there a problem with the drive?

Have just returned from a cross-town mission to Carshalton and (touching Ikea fake wooden desk) no problems. It seems this happens in heavy traffic where there is a lot of stop and starting.
could be theres a little bit of slip in traffic which is heating it up too much, sometimes if a clutch gots too hot it can start juddering, had that a few times - usually its fine again once it cools down, unless you slip it really badly (ie smoke coming off it)
It could be oil on the clutch face, a warped plate, a damaged fly wheel, a damaged pressure plate, knackered springs in the the clutch plate or maybe even a broken engine mount. All these could be caused by bad driving technique or faulty parts apart from the oil which would be a faulty part.
