What decides when to install a footbridge?

anyone know the decision tree on when to install a footbridge?
Is there a figure for how many vehicles a day when it's ok to have just a gap in the central barrier for pedestrians to cross a dual carriageway?

Cant really help with the facts and figures but I expect someone has to get killed (usually a child) before a footbridge is considered.
A prime example is Jades Crossing on the A249 in Kent.
Locals had been trying to get a bridge installed for years before the inevitable happened..

Thanks for the link, I'd driven past there a couple of times recently (due to M20 roadworks) and wondered if there was a story behind the bridge due to the name and ballet dancer silhouette.
