Turbo to manifold nuts loose

For the second time recently, the nuts holding my rear turbo to the manifold have come loose. Last time, the turbo had been fairly recently replaced and so I assumed it was just a case of needing a tweak after new gasket had bedded in etc.
I have some questions:
1) Is there a likely reason why they may have come loose a second time (apart from them just not having been tightened enough - it was done trackside by a friendly mechanic)?
2) Is there any point in loctite or similar, given the heat that's generated there?
3) Any tips for getting the best access to the nuts so that I can ensure that they are tightened properly this time - what direction do you go in from. Is it best with clam off?

Philidas do a double bent beam turret nut which are ace for this sort of application. If you can use an EZP or BZP bolt too that helps in the higher tensile (12.8) variety. I use a 1.5mm copper gasket which personally I think is better than the steel ones because you can get a good bite and a decent seal around the whole circumference of the port.
