Hi,

Can anyone brief me on the procedure in the UK for getting one of these?

Go to reasonably large Post Office.
Fill out form.
Get it stamped.
Put with Passport etc

Take photocopies as well - docs and hospitals tend to want to keep it for their records, and it's better if you can give them a copy, and you still have more to carry on with if you injure yourself again!

Oh and forgot to say that if you're going to fill it in 'while you wait' at the post office, the ID number they require on it is your national insurance number, so take a note of it with you.

...and make sure you get a new one. If you had one last year it is no longer valid. You need to take two forms, and it is best to download them as most Post Offices in this neck of the woods (Bristol) had run out of the paperwork when I applied last month.

And guess what? the new system is only valid for this year, a new Euro health i d card comes in next January (cue loads of eurosceptic criticism!, but let's keep this thread to the point.)

They'll also accept an NHS number if you don't have your NI number to hand.
