The car is a 987.2 manual.

The gearstick makes a very noticeable rattling noise and vibrates when accelerating in 5th gear.

Would this most likely be engine or gearbox mounts, gear linkage, or something relating to the gearbox?

It seems to be even worse when accelerating at higher RPMs, but as far as I can tell it only happens in 5th gear.

If your exhaust pipe is sagging this can indicate mounts are sagging. I think mine has a saggy front mount as the rears looked fine when I removed them. But I've not heard anyone suggest saggy mounts cause vibration. I had some weird shifter clunking when my 996 gearbox internals were on their way out. But this was only noticeable coming on and off the power. I'm genuinely stumped here, sorry.
