Changing C1 provisional to a C

Hi all,
I have had a good google but am struggling! I have a provisional C1 and was looking at doing the C1 to enable me to drive a 5t van for work however I have come to the conclusion I might as well just do the C. How do I go about upgrading from the C1 to C provisional? Everything I can find assumes you have the full C1 or nothing. Do I have to do the medical D4 again (I did this last year)?
Also with regards to the theory I cant find any of the multiple choice to book locally but can find the hazard perceptions. I assume so long as its no longer than 2 years from taking it to passing everything there can be a gap between the HP and the multiple choice?

I would send off the D2 form with both C & D ticked and see what happens

Each separate part of the theory lasts for 2 years
If needing the licence to drive commercially then you will also need module 2 cpc theory followed later by Mod 4 practical
