I'd never read the whole thing. Sobering but also surprisingly moving to think someone had to sit down and write that.

Luckilly, never required.

Although when they did suffer disasters in the Shuttle programme, they were taken by surprise.

Well according to Mike Mullane Challenger was a “predictable surprise” so maybe a preemptive letter should have been written for the president for the shuttle programme

There were lots of memos and letters flying between various agencies, departments and contractors in the lead up to the Challenger accident. Many people knew there was a potential problem. That's why Mullane classifies it as something that was preventable.

In fact, his favourite term is "normalisation of deviance" - and he now does the lecture circuit giving talks on this phenomenon.

His book "Riding Rockets" is an excellent read.
