Why was he let down by so many doctors?

Basically my work colleague of 11 years passed away on Monday 3rd January 2011.
He had been ill for 10 years & gradually got worse every single year. He had seen numorous amounts of doctors etc but never had his illness diagnosed until 7 months ago where he was told that he would need a liver transplant but due to "poor" health they wouldn't be able to operate until he was phsically stronger. He was then told on December 30th that there was nothing more that could be done & he would have a maximum of 1 year to live. My whole point to this is why wasn't this spotted earlier & why did they let him get to such a stage where nothing could be done. I feel as if he has been let down very badly & all of this could have been prevented. Surely these day's such things could be spotted alot earlier. RIP Richard, no longer in any pain.
