Even though I have a relatively simple medication regime (1 pill per day), I regularly struggle to remember whether or not I've taken it.

On holiday I tried one of these pill boxes with 7 compartments and the day of the week on the lid of each. Not really that good. The day of the week rubbed off, and opening each compartment risked your nails.

There must be something out there - any recommendations?

I use the medicine reminder stuff on the iphone. pop in your drugs and when you take them, and it'll remind you, then nag you, and record whether you have or not (as you tell it you did).

I use a 7 day compartment box. Days are raised on the lids, not printed, and the lids open without use of nails. Try a different box.

This. There are several on Amazon with a button or tab you push to flip the lid up. I have this one:

I take 4 pills in the morning & 2 at night. I find the key to remembering to take them is establishing a routine.
Pills in the morning - boxes in small tupperware container on low shelf above where I eat breakfast. Take pills after eating when finishing off glass of juice. Never forget.
Pills at night - boxes by toothbrush. Never forget.
However, if I'm away from home!

I take pills three times a day and have a week's worth in each of two holders. The days are on the holder rather than the individual pill containers so I can see at a glance which day's to pick up; similar to this:

If you’re taking one pill per day I don’t think you need anything other than a pen and your pills. Write M, T, W, T etc on the blister pack and stick it on your bedside table, next to your keyboard, where you keep your car keys etc. if you’ve not taken one it’s obvious.

Marking with a pen as mentioned above or ticking off each day on a calendar or diary as you take them should work.

Surely though when you don't get an erection you must know you have forgotten.

I always have an erection.

As said, routine. I've set an alarm on my phone that goes off at the same time every day.

Is that a sign you’ve taken a pill or you need to take one?

I missed the 'one a day'.

Record 'Take your pill!' on your phone and use that as your alarm sound at whatever time you have to take it.

Excellent service and free. Comes in dated pouches on a roll (think supermarket queue ticket). Used them for a few years now.

My wife uses the 14 box ( 7 am and 7 pm compartments per day ) from Amazon - think it was £6.
