Before I spend money at a physio , I thought perhaps I might elicit some advice here .
For a week or so I have had pain on the left side of my left kneecap.
The pain is in the form of a stretching/tearing sensation of varying intensity , the pain seems to go across the front of the knee , walking the pain is mild , stairs are ok up, but down painful to the point of having to do one step at a time , laying in bed the joint seem so stiffen and movement can be very painful at times although once I get out of bed ,just mild until I have to negotiate stairs.
I can’t take ibuprofen due to other meds , paracetamol only gives limited relief .

Might get an appointment in a couple of months that’s how long I am having to wait to get a diabetes review .
And when I eventually get an appointment , I have no doubt I will be referred to a physio , so more waiting !
And I do not have time to sit in A&E for 12 hours

I had Achilles tendonitis last year, went to the doctor, put me in for a scan.....in the meantime I found some exercises online and fixed it myself before the scan appointment came through about 3 months later. But without the doctor's diagnosis I wouldn't have known I was doing the right thing.
ACL is a serious issue, I'd be pressuring the doctor for a diagnosis

You may have tore your ACL or another ligament, may have even tore the Meniscus. You really need a scan though and to see a doctor to be sure.
I did my ACL years back playing football, luckily I have private medical and was seen to quickly but did require an operation.

Have you looked into Piriformis syndrome?

The reason I ask is I had severe knee pain only when running downhill, and it was my piriformis.

What you describe isn't ACL or PCL, the cruciates.
Most knee problems can be diagnosed by a targeted and experienced history taking (a chat) and an experienced examination.
MRI misses 1 in 20 things (false negatives) and finds the wrong thing in probably 1 in 20 (false positives)
People think MRI is perfect.
Scan alone isn't the answer, sometimes I ignore it, sometimes I don't get one.

You need an experienced knee healthcare professional, be they physio, surgeon, GP or whatever.

Tore my ACL in 1993, twisted on it whilst playing football (foot stuck in grass whilst turning, knee joint just twisted out). Knee stability was awful so much so that I could sometimes collapse over and also problems coming down stairs as you say, going up was fine. Had surgery on the NHS and was lucky that I got a good surgeon, minimal scar on my knee vs some people I have seen that look like they have been attacked by a shark! I am sure it will be far far better in 2023 though.
