Fit to work certificate - getting a private one.

Just been assigned a nice project from an international public institution. Part of the contract requirement is that I provide a Medical Certificate to state I'm fit for work. I don't need an examination - just a doc to say, yeah, all good.
My surgery would normally do this but are rammed obviously and have quoted a wait of 4 to 5 weeks.
I've found a few online resources that can do this privately but this results in a letter that they generate - I need someone to complete, sign and stamp the form.
Any suggestions and recommendations would be welcome.
