I have a brace of Yorkies, combined total 6.5kgs - they're pretty small.
Regular toothbrushes are just too big, and the doggy ones that fit over your finger are also too big.

Stella's just had five extractions and I don't want her or her brother to go through that again.

Can anyone recommend a small toothbrush or anything else suitable?

The Virbac toothbrush is double ended, and has a pretty small head at one end from what I remember:

I think they also sell these at P@H.

We were recommended Plaque Off, i was sceptical but seems to work. Vet confirms no dental problems of teeth cleaning req on 8 & 10 yo.

I use it for my 2 dogs & 1 of my cats, it was highly recommended by the rescue places I adopted the dogs from.

Combined with regular brushing (no, not the cat!) it seems to work well.

No idea on smaller brushes though, my 6kg Westie is fine with a normal brush. Can you use a kids brush?
