This came off an automated blinds system at my parents' house.

There are 12 blinds which are all powered from one central controller, but two of them have stopped responding to the controller. A cable runs along a track and each blind has one of these boards plugged into it. The motors on the two problem blinds will move if I plug them into one of the other boards, so the motors themselves seem fine.

The system has been fitted since 2014 and the company have said they will only cover warranty for ten years, but the rest of the system is fine so it seems overkill to replace everything when it (mostly) all still works. I'd like to try replacing this board for the two problem blinds.

Judging from the marks and discolouration on the right hand side, I think moisture has got to this board and killed it. But Google has no idea what this board is or where we could get a replacement. Can anyone recognise the board or suggest a possible source for a replacement?

It will be a custom board by whichever company made the controller. Why not ask the company that supplied it?

Looks like it's control signals coming in on the R, motor power out on the L. A short (or water ingress) on the left has probably damaged the output stage. If you can read the labels on the big transistor you could try just replacing that.
