Not sure where you are off to, but on my visits to Tobago and Antigua the locals all preffered US, they weren't keen on EC unless a last resort. When I went back the following year I hardly took any EC at all.

I got my EC delivered from my bank both times, didn't bother shopping around to be honest.

As above, in my experience they're happy to take the US Dollar.

Its Antigua.

For the Visa on entry is US $ fine too?

How does the pricing work when you are there i.e. is the Eastern Carribean pegged to the US$.

It's been a while since I was there, but I think prices in restaurants and the like are quoted in US$. Can't remember paying a visa on entry.

We were followed around for a while by a one-legged man who wanted us to give him money however.

Also, the taxi driver from the airport got stroppy because I didn't give him enough of a tip.

That's all I really remember.

Depends where you are in the country I suppose, but I always use this place http://www.cityforex.co.uk/index.cfm

US$ is accepted pretty much everyone, but I seem to recall that it was slightly more advantageous to use EC$. EC$ - US$ rate is pegged at 2.7 - see what rates you can get for US$ and EC$ and work it out from there.
