Why don't they just call him a Carnival King?

Why do they need to call him the Carnival King just because a few knuckle draggers can't help but make threats and abuse the organisers?

I don't understand the threats myself but the answer to your question is :

"It's the tide. It's the dismal tide. It's not the one thing".

I don't really want to live in society where 16 year old boys are dressed up or presented as queens and sent on a parade. Other people here might be ok with it but it would be my personal preference that this is not normalised.
