4R100 Torque Convertor destroyed

After a nightmare 5 months without my Big Bertha (Ford Excursion) due to gearbox issues I have finally had news that my torque convertor has been utterly destroyed.
How on earth could this have happened? Was it possibly a lack of fluid, or something that has caused this? I don't drive her hard. In fact, I've never babied a car so much as I do her.
Anyone have a spare 4R100 torque convertor?!
