Evening Gents.
You guys out there, are you familiar with BCI Basic Cause Investigation?

Most companies have this, usually generated from a significant event , which leads to a tracker, which leads to someone who is randomly selected ( particularly in this case) to look into ) probably to be objective and impartial.

Where do you start?

The fickle hand of fate has pointed me out to look into this.

The IP had a minor injury, but it could have been much worse.
In many aspects.

Usually everyone will lie to you.

I once had someone swear blind he wasn’t smoking in a non smoking area despite knowing we had full cctv and access card coverage.

If you've had no training or prior dealings with this, I'd politely inform your manager that they either need to find someone else to do it, or give you some training on what to do.

You need to get HR or line management to give you full details of what is expected and how to proceed IF they want you to do so.

I would advise that in order for the investigation to take place you need to have had some form of training.

If it’s for root cause to stop it happening again it can be a lengthy process and you need to have a good framework in place.

If you want to have a open conversation or for me to send you some reading material, drop me a message with your email address. Can always run through an outline of what a good investigation looks like on teams.

In my current business, anyone undertaking an accident investigation would have completed an incident investigation course as a minimum and would shadow an experienced investigator until they felt comfortable.

Interviewing a person to gain factual information is an art form, to get truthful answers there needs to be a no blame culture in place with no repercussions to those involved in the incident.

I'm a qualified accident investigator for accidents and incidents on the Underground, I have done a couple, I really wouldn't want to do another one. If you do it properly you make far too many enemies.

Thanks guys.

Sorry I’ve not replied in a while.

I’m currently on holiday in Spain , nursing an absolute humdinger of a hangover.

I’m trying to put this investigation behind me.
I’m taking on board comments. I work in a heavily regulated industry.

I’m thinking I’m being set up for a fall.

A senior operations manager asked me a few week ago.

‘This investigation you are looking into? How bad is it as (I’m worried)

I replied, with a straight face. In my experience, If it looks like a turd, smells like a turd - it’s probably a turd.


And it’s a turd of epic proportions.

I’ve had some training, admittedly it was over 10 years ago.

Probably not much use now. But I have a probing mind and years of experience. it’s become really obvious to me that whatever I report won’t look good at all.

I think I’m going to duck it and say I’m not up for it.
