Motorist fined for causing a cyclist to fall off...

I'm sort of confused here, ok, vehicle passes the cyclists, but he is ns wheel on edge of road and not exactly tanking it. The fall appears to have been caused by cyclist #1 stopping to remonstrate, and cyclist #2, applying brakes and not unclipping her feet? Had she been competent to unhook her feet, she'd have not fallen, and presumably if cyclist #1 have not stopped, she'd have not fallen too. Did he need to stop..? IMO, not really.
We have all fallen foul of the evil cleats when starting, what i dont get is why it is anyone's fault except the operative!
