Players running into the penalty area before the ball was touched I think.

Yep, that was my take on it.... and then the keeper advanced off his line too.

Have no idea why the header was disallowed!

That's what I thought to begin with but I replayed it and they were far from entering the area; especially after penatly number 3 i think it was!
