very strange fault on C4

Diesel C4 by leob front end shaking depress clutch and it gets worse, engage gear then release pedal starts shaking like anything take it out of gear and you can't Select gear again, without a real struggle, and crunching. Any ideas at all lads? its got me baffled

Unbalanced/damaged/off centre flywheel or clutch plate?

I have no idea how it could get like this. But it's the only thing I can think it could be.

Apparently its a duel mass clutch and its a common fault on them ???? Skodas do the same. Thanks for the reply slomax.
