If you got two separate gears, the smaller one double thickness, you could turn half of it down to be a plain spigot, and bond it into a corresponding hole bored in the larger one.
It sounds like a double gear that you want.

I would be amazed if you could get anything from the catalogue exactly what you require.
Specify exactly what you want, and they will make it though. If you want a 1-off, it’s going to be expensive, so tell them what you can compromise on to get the price down.
Does it have to be plastic?
Most of the gear turnover they do is bespoke, and the catalogue is a good guide for bespoke prices. Well it was when I worked there in the “design office”, although that was in 1985!
