Engine Problem Volvo V70 D5

Hi All,
I am posting on behalf of my brother who is having problem with the engine of his volvo.His description is as follows:
I got back from a 200 mile journey this morning and went to turn the car around and noticed that it is making a strange noise.
The engine sounds like it has a blow in the exhaust but I have been underneath and inspected and there is nothing wrong. On removing the oil filler cap it sounds like there is a strumming noise from inside the engine and my first thought is broken piston rings or bearings. I hope I'm wrong! I have been told that injectors can cause knocking but am not sure.

The car is a 2002 V70 D5 (D5244 engine), it has covered 225000 miles and other than the noise at idle it drives perfectly. I have checked the MAF, replaced the fuel filter, shoved through some injector cleaner and changed the air filter.
The oil is not new but is at the correct level.
