Pay Rise/Annual Review

I am coming up to the end of my first year with a new employer and I want to ask for an annual review/pay rise.
When i started, not objectives or anything was really set for me. I just got on with the job and have loved every minute of it. The company are doing really really well - when i started last year their was 6 employees now there are 12 with more starting on a pretty much monthly basis. The company also has plenty of dollar in the bank i believe.
I feel like i have really pushed myself this year and have given atleast 110% and done many things that arent part of my job description for example, flying down to london last saturday to train some end users (not part of my job at all, but was good fun).
How do i ask for an annual review/pay rise?
Should i just ask for an annual review to set some objectives for the next year and then take it from there? Please help.

Sorry.
There is a bit of a wobble in the economy at the moment - if I was running your company I would probably not appreciate being asked for pay reviews at the moment. Companies will want to sit on their cash as the banks aren't lending - therefore cashflow is tight.
You would probably be best advised to ask for a performance review and see how they rate you.
