How The Hell Does This Happen?

About three months ago, my MR2 blew up.
Yesterday, I got the car back, new and improved with a turbo lump.
The old block has four holes in it, two of which are as big as your fist.
You can see daylight through one side of the block and out the other.
One of the piston heads has been reduced to about a quarter of its original size.
I was in fourth gear when it blew, accelerating from about 60mph and having trouble getting the car to go anywhere... It blew at about 75, with about another 2.5 thousand RPM before the redline. And those 3SGEs are rev-hungry engines...
It's not like I missed a gate going from fourth to fifth at 100 and accidentally slipped it into third, so how the hell could so much damage have been done?
