I know you have said Hatton Gdn, but there is a guy called Peter Martin in Southend, who has done a lot of bespoke work for members of my family. He will sit with you, discuss ideas and draw a design, before making it.

Don't know where you are based, but you could consider the jewellery quarter in B'ham ... I'm told that the prices here can be better than in Hatton Gardens ...
