She was on the M4 in her Fiat 500, and said she pulled over as soon as she heard a funny sound, I wonder whether the pressure may have been low before this happened, any thoughts?

It looks fairly low on tread, it could be low on pressure putting more strain on the shoulder of the tyre it heats up and fails, but that is some damage for the sidewall to completely fail.

Looks like it's had a hit looking at the rim. Has she carried on on the runflat until it's failed??

I can only imagine that's down to age and it's finally blown, or if it's been repeatedly bounced off things in the road like kerbs and what not to weaken the side wall. Odd failure though.

Looks like something stuck in the tread corresponding with where it has split too. Possibly not the root cause.

Pinched against a kerb at some point or pot hole strike breaking one or more sidewall bands a good possibility, there might have been a bulge developing as early warning.
It doesn't look as if its been runflat, but if thats the case it will show up better inside.

Scary experience for a her a rapid deflation.

Judging by the (what looks like further) lateral damage across the tread & the distortion around the rip I would suspect that has been severely weakened by a pot hole or kerb strike.

Agreed, that wheel needs replacing as well, that is very distorted.
