Ford Focus stalling problem

I drive a Ford Focus Diesel (year 2001). It stalls at low speeds - especially when I'm slowing down in third and I put my foot on the clutch. It only happens intermitently - sometimes once a week, sometimes twice a day. It doesn't seem to be caused by weather, and it starts again straight away. ANy ideas how to fix it?
