Parking ticket quotas.

As some of you may have seen in the news there are various issues surrounding parking tickets -
without going into details there are other important matters happening which may have far reaching consequences, and I could do with some assistance.
Does anybody have a Parking Ticket Reminder Notice from any council (except Christchurch!) that I could have a copy of? I'm not interested in details of the recipient or the alleged offence, just the dates shown on the front and the wording of the paragraph 3 on the reverse (under Further Action).
