The Main Company (It's not called Main Pine.) in Green Hammerton.
They've done my kitchen, dining room, bedrooms, office, hall and even the oak on one of my garage blocks. I went for oak throughout.
Croft Builders, next door to Main, run by Wayne Croft (top man) gutted the kitchen and dining room and changed the layout ready for Main as well as fitting underfloor heating, tiled floors and various other bits to change the shape. They also built the quad garage block I mention above. Croft also gutted the original triple garage attached to the house and created what I asked for exactly.
Superb work throughout by both companies and they co-ordinate well together. I found I had to manage things a bit with Main from an organising point of view but they were superb over the 3-4 years we worked through the house and have been equally good supporting with any minor issues afterwards.
As said above, not cheap but not expensive for what it is (i.e. not expensive because they're more than other companies but because they only do quality stuff). My view is do it once, do it properly. We're staying here for another 20 years and the most I'll do is paint the bits we had painted. Nothing breaks as it's rock solid. Bespoke too so it's what we wanted that fits our house.
We may have felled a forest or two though. Oops. ;-)

I think it goes without saying that a completely bespoke kitchen isn't going to be on the cheap end of the market.
