With respect to ripping the nut out of the body:
Is there evidence, that the hinge bolts are coming loose, if their heads are not touching the hinge itself.
When I was to grease the hinges, I noticed this and I'm amazed as the door itself has no freeplay, which should be typical if hinge bolts come loose.
