Vectra ECU problem

I have a Vectra LS 1.8 16 valve 18XE1 1999. The ECU has packed in according to Vauxhall dealer. I need to find a replacment if any one can help.

Part numbers as they are on the sticker

GM 90 569 370
S 99 001-04
SIEMENS 5WK9
9 08 12 06 09 0 1706

Question is which numbers do i need to match? Is it just the top 2 lines or is it more? Any help or advice is much appreciated.

The GM number should be enough.
